Community Voices Share our Work

At Yamhill Community Care (YCCO), our mission is to work together to improve the quality of life and health of the communities we serve by coordinating effective care. We envision a unified, healthy community that celebrates physical, mental, emotional, spiritual, and social well-being.  

Our Community Advisory Council (CAC) embodies these values by bringing together local voices to guide our efforts. We know that the best way to improve health and well-being is by listening to the people who live in our community. That’s why our work is guided by members like you—people who understand what it’s like to navigate health care and want to make a difference. 

What is the CAC?

Our CAC is a group of local community members, including Oregon Health Plan (OHP) members, caregivers, advocates, and community partners.  

What does the CAC do?  

One of the CAC’s key responsibilities is developing and overseeing the development and implementation of the Community Health Improvement Plan (CHIP) (en Español)—a roadmap for addressing health priorities in Yamhill County. The CAC also allocates grant funds that support projects that bring this plan to life, making a lasting impact on our community. 

Community-Led Projects 

The CAC supports a variety of programs that promote health and well-being, such as:  

  • Ensuring access to healthy food for kids and families 
  • Making sure people can access healthcare in their preferred language  
  • Programs to help people plant gardens 
  • Oral health education 
  • Increasing activity and nutrition for kids and families 
  • Improving access to transportation  

 These initiatives align with YCCO’s commitment to learning, listening, changing, and growing.  CAC plays a crucial role in this continuous improvement by providing insights and feedback from the community. 

The CAC reviews and provides feedback on developing YCCO policies and projects and provides recommendations to the YCCO Board of Directors. Their insights help shape the health plan and ensure that our programs reflect the real needs and priorities of the people we serve. 

Who can join the CAC? 

The CAC welcomes all voices, including OHP members, caregivers, people with lived experience, and representatives from local organizations. We value diversity and are open to people of all ages, backgrounds, abilities, and perspectives. 

How often does the CAC meet? 

The CAC meets monthly to discuss the health needs of our region and provide recommendations to the YCCO Board of Directors.

Meetings:
Second Tuesday of each month

Time: 5:00 p.m. – 7:00 p.m. 

Where: Yamhill Community Care Board Room,  819 NE 3rd St, McMinnville, OR 97128. 

Services: 

  • We provide simultaneous translation 
  • We provide childcare 
  • We provide dinner and are happy to accommodate dietary needs 
  • We offer hybrid options via Zoom
