Community health is more than seeing good doctors. When someone needs an advocate at an appointment, doesn’t know where to get food, or has complex health conditions, we’re here.

Community Health

More Than Just a Health Plan

Health is more than treating illness. It includes physical activity, eating food that’s good for you, and having a safe place to sleep. It includes having everything you need for your child to start school, a good social support group, and services in your language offered in a way that works for you.

Yamhill Community Care (YCCO) is invested in not only treating symptoms, but preventing them before they start and improving whole-community well-being. A stable, supportive environment makes everything healthier!

Community Programs that Make a Difference

Yamhill Community Care works to provide quality community programs that address the Social Determinants of Health (SDoH) for Members.

Social Determinants of Health chart

The World Health Organization describes Social Determinants of Health as the following:

“Health starts in our homes, schools, workplaces, neighborhoods, and communities. We know that taking care of ourselves by eating well and staying active, not smoking, getting the recommended immunizations and screening tests, and seeing a doctor when we are sick all influence our health. Our health is also determined in part by access to social and economic opportunities; the resources and supports available in our homes, neighborhoods, and communities; the quality of our schooling; the safety of our workplaces; the cleanliness of our water, food, and air; and the nature of our social interactions and relationships.”

Community Health Hub

Sometimes getting the care you need is not easy. Keeping appointments, finding programs to help with food or housing, and meeting your goals can be hard to do. The great news is you are not alone!

Our Community Health Hub team will partner with you to live the healthy life you want to live. They can help in whatever way you decide.

Traditional Health Workers

If you have any barriers that prevent you from keeping good health, Traditional Health Workers (THWs) can work with you one-on-one. They provide some health and mental health services. They also connect you with local people and resources.

  • Help you get a provider for your dental, physical, or mental health
  • Health schedule health visits and go with you if you choose
  • Share about resources in your area, like food or rent assistance
  • Work with your doctor’s office or social services
  • Talk to you about your YCCO health benefits
Traditional Health Workers

Local Integration Family Team (LIFT)

LIFT is made up of local people, who come together to gather supplies and services to meet immediate needs for people. They also help build long-term health and wellness for each region.

By intervening early we can achieve community-wide wellness for generations to come. That’s why we support programs that are proven to encourage a positive way of life and give people the tools they need to stay healthy.

Parents have one of the hardest jobs in the world — and also the most important. We are here to support you as you prepare your child for success in school and in life.

FamilyCore Program

FamilyCore is a community services network connecting parents with a broad support system to meet challenges head-on and to nurture strong families with children ages 0–5.

FamilyCore can receive referrals from providers or patients. Use the form to submit a Yamhill FamilyCore referral or request services.
