McMINNVILLE, Ore. — Yamhill Community Care Organization emerged out of a grass roots movement committed to transforming how healthcare is delivered in Yamhill County, based on local control and accountability to the community. As part of this continuing commitment, Yamhill Community Care will transition the management of its medical care innovation work to its McMinnville offices.
To provide clinical leadership of this work, Bhavesh Rajani, MD, MBA will join Yamhill Community Care in its newly expanded position of medical director. In this position, Dr. Rajani will play an integral role in Yamhill Community Care’s ongoing work of facilitating the collaboration of local medical care providers to implement practices that ensure the best possible outcomes for patients. He will also be a key leader in the expansion of the CCO’s Wellness Center programs (currently consisting of the Persistent Pain Program) and registered nurse care management services.
Dr. Rajani was hired not only because of his many years of experience in clinical practice and senior health plan management positions, but also because of his demonstrated passion for serving the most underrepresented and vulnerable members of the Yamhill County community. This dedication comes from his own personal and professional experiences.
Dr. Rajani grew up in Africa and moved to the U.K. as a refugee at a young age. “We had very little for many years, and I learned that being in those situations is often out of your control,” said Dr. Rajani. “But I also learned that when people are given chances in their lives by having good healthcare and by having their basic needs supported, they are able to focus on developing a successful future.”
Yamhill Community Care has already benefited from Dr. Rajani’s holistic and community based vision for healthcare transformation in his early work with its Quality and Clinical Advisory Panel. He also represented Providence Medical Group on the CCO’s governance board.
“Yamhill Community Care Organization is leading the way in our nation to create new ways of providing access to healthcare,” said Dr. Rajani. “I am excited to help drive the work in striving to improve the quality of care, and in continuing to serve the Yamhill County community.” Dr. Bhavesh Rajani began his new position on May 9, 2016.
