Yamhill Community Care Awards $3.6 Million to Local Organizations
Grants support 29 organizations addressing social determinants of health

McMinnville, Ore. – Yamhill Community Care (YCCO) recently awarded more than $3.6 million to 29 local community-based organizations through its Joint RFP and Community Capacity Building Funds (CCBF).

The Joint RFP supports local organizations in addressing the social determinants of health and equity in Yamhill County. The Joint RFP funds include the following funding streams:

• Community Prevention and Wellness (CPW) Fund
• Supporting Health for All Through Reinvestment (SHARE) Initiative
• Early Learning Hub (ELH) Fund, Traditional Health Worker (THW) Fund, and the Community Health Improvement Plan (CHIP) Fund

The CCBF funds support local agencies’ ability to provide climate devices, housing support, nutrition assistance, and tailored outreach and education to YCCO members through the Health-Related Social Needs services.

The selection process involved detailed reviews by the Early Learning Council, CPW Committee, Community Advisory Council, comprised of community members, using rubrics to evaluate alignment with each program’s criteria. The YCCO Board gave final approval as required by the organization’s bylaws.

Joint RFP Grantees:
The Bridge Learning Community: Support community gathering space that facilitates an enriching, supportive experiences, and spaces for children and their caregivers.
• Capaces Leadership Institute: Support for cancer prevention and OHP navigation for Spanish-speaking and farmworker communities.
Churches United for West Valley: Provide child friendly food items to families with insufficient food provisions.
Community Wellness Collective: Traditional Health Care Worker funding and support to ensure vulnerable individuals receive case management and coordinated care for supportive services and partial funding for low-barrier drop-in center renovations.
Creating Opportunities: Partial capital funding of land in the Yamhill CCO Service Area to build a welcoming and accessible Community Resource Center to provide office space and a large flex space with a commercial kitchen for community meetings, educational workshops, and social gatherings.
Dayton Community Food Pantry: To continue funding for 3,000 sq/ft dedicated facility to increase the Food Pantry’s capacity.
• George Fox University: Promote professional development and academic success among bilingual high school and college students with bilingual engagement, culturally oriented mentorship, and empowerment program.
Grand Sheramina Community Services Food Pantry: Infrastructure funding for large service pantry in Yamhill County.
Halloran-Steiner Mindfulness: Traditional Health Worker funding and support to learn to live and work in a mindful way to enhance their coping with stress and improve the care they give to those they serve.
• Henderson House: Support the Survivor Housing Project to eliminate barriers and improve access to housing resources for some of Yamhill County’s most vulnerable residents.
Juliette’s House: Child abuse prevention education program for students from pre-K to 12th grade, and adults who work with them.
Lutheran Community Services: Capital funding to make vital physical improvements to the mental health care space.
McMinnville Area Habitat for Humanity: Support low-income home builds.
McMinnville School District 40: Support the Social Emotional Learning and a Multi-Tiered System of Support program in schools.
McMinnville Playschool: To hire additional staff to support students to meet their development milestones.
Mother Tree International LLC: Traditional Health Worker funding and support for advanced evidence-based training in improving perinatal behavior.
• Newberg School District: Implementing RULER and BARR Model within Newberg-Dundee Public Schools, this evidence-based intervention improves health by changing the impact of social and economic determinants on health and providing resilience factors within our school community, buffering behavioral and medical health risks.
Oregon Family Support Network: Respite for families with children with disabilities.
The Soup Kitchen at St. Barnabas: To continue serving the most vulnerable with nutritious food in Yamhill County.
Unidos Bridging Community: To implement a nutrition program in Spanish for Latino immigrant and agricultural working families and individuals, focusing on heart disease and diabetes prevention and education on nutrition, healthy eating, and physical activity based on the U.S. Dietary Guidelines for Americans.
Yamhill County CASA Program: CASA volunteer recruitment and retention support.
Yamhill Enrichment Society: Support program to promote the social and cognitive development of preschool children and provide barrier-free access to high-quality books that help families spend valuable time reading together, create home libraries, and prepare children for future academic success.
Yamhill County’s Nutrition Oregon Campaign: Expand the Veggie RX pilot to 50 pregnant people through Yamhill County Public Health and provide three meals a week for up to four members of the household for six months to find the best method of implementing program in Yamhill County.

CCBF Grantees:
Community Wellness Collective: Supporting housing, nutrition, and outreach to individuals in Yamhill.
Creating Opportunities: Support housing, nutrition, and outreach support for individuals with developmental disability in Yamhill and Polk County.
Helping Hands Reentry Outreach Centers: Support individuals with housing navigation, transitional housing, shelter, and resources in its service areas.
Henderson House: Support housing, nutrition, and outreach to individuals in Yamhill County facing domestic violence.
Just Compassion of East Washington County: Prevention of houselessness and develop frameworks to house YCCO members in the community in Yamhill and Washington County.
Meals on Wheels: Expand access to meal delivery and nutritious food through support Health-Related Social Needs support.
Oregon Wellness Network: Support for climate, housing, nutrition and outreach for seniors and people with disabilities in its service areas.
Polk County: Support Polk County residents in navigating climate, housing, and nutrition outreach.
YCAP: Support staff in trainings and health-related social needs outreach for Yamhill Couty residents.
