Local Grassroots CCO Ranks Among Top Five in the State of Oregon
McMinnville Oregon, October 2020 –Yamhill Community Care and other Coordinated Care Organizations (CCOs) across the State of Oregon received the final report for the 2019 Oregon Health System Transformation Metrics. This report is a description of the progress of Oregon’s CCOs on quality health measures. The report is distributed by the Oregon Health Authority.
Out of the 15 CCOs in the state, Yamhill Community Care was ranked among the top five highest performers by meeting 100% of quality pool funds as well as the four challenge pool measures. YCCO met a total of 16 out of 19 health measures. Among the most notable measures were those related to health services that impact families and children. The percentage of Yamhill Community Care members who received recommended childhood immunizations rose by more than two percent since 2018. Members receiving prenatal and postpartum care also increased by more than two percent. In addition, members under the age of three who received required developmental screenings increased by 3.5%, and children in foster care who received necessary check-ups increased by an impressive 15%.
“These results show how grassroots coordinated care organizations are making a difference,” says Seamus McCarthy, President/CEO. “Four of the five top performing CCOs are smaller, community-based organizations like Yamhill Community Care.
Meeting these health measures means YCCO receives additional funding, allowing for financial support for community-based programs that impact population health. The programs are assessed for their likely impact on YCCO members and the community as a whole. Organizations that receive financial support for these programs include school districts, food banks, health care providers, social service organization, among others. The work that went into achieving these metrics was a community effort across all health care sectors. It is a testament to the value of the coordinate care model, especially models that are built on the concept that all health care is local and that community governance of these models is key to achieving better health outcomes.”
