McMinnville, Ore. — Yamhill Community Care Organization (YCCO) is pleased to announce a new collaboration with Performance Health Technology (PH Tech) and Providence Plan Partners (PPP), effective January 1, 2019. YCCO is responsible for the care of Oregon Health Plan members in Yamhill County and surrounding areas. PH Tech and PPP will work with YCCO to build a YCCO specific provider network, help with care management and provide Pharmacy Benefit Management.
Oregon’s Coordinated Care model was developed in 2012 to coordinate Medicaid services – including physical, behavioral and oral health needs — to provide better care to more people at a reduced cost. In 2019, all CCOs will be required to re-apply to the Oregon Health Authority for another five-year contract and will need to demonstrate their ability to meet enhanced expectations in four areas: maintaining sustainable cost growth, increasing value-based payments, focusing on social determinants of health and equity, and continuing integration of behavioral health services.
This new partnership is part of Yamhill CCO’s plan to address these expanded requirements by enhancing care management services and ensure long term sustainability. This transition will also allow for improved metrics tracking, enhanced data integration and increased collaboration with local providers.
For YCCO members, coverage will not change. Most members will continue to be served by their current primary care providers, although some reassignments may occur. Communication about this new partnership was mailed to all YCCO members in mid-November. New ID cards will be mailed the final week of December 2018.
