Yamhill Community Care Seeks Community Proposals for its SHARE Grant Program

Applications are due September 15

McMinnville, Ore. – Yamhill Community Care (YCCO) is seeking community proposals for its SHARE grant program to support health for all through community reinvestment.

The Supporting Health for All through Reinvestment (SHARE) grant program aims to create positive change by reinvesting funds in community initiatives that address health inequities and social determinants of health and equity (SDOH-E).

Applicants are encouraged to align their proposals to address our community’s specific needs within SDOH-E and with the community priorities from Community Health Improvement Plans (CHPs). The CHIP priorities include housing, food and nutrition, infants and children, mental health and substance abuse, emergency preparedness, access to healthcare, and transportation.

To be eligible for funding, applicants must be 501(c)3 organizations, Tribes, Urban Indian Health Programs, or public agencies. Preference will be given to projects targeting historically or systemically underserved populations, including ethnic, linguistic, economic, and academic disparities.

The application process for the SHARE grant program is now open, with a submission deadline of September 15, 2023. Applicants can apply online or submit completed applications via email or mail.
