Yamhill Community Care Invites Community-Based Organizations to Submit Proposals to the 2024 YCCO Joint Funding RFP

Joining Forces to Support Local Organizations and Amplify Impact

McMinnville, Ore. – YCCO has released two separate funding opportunities: 1). A multi-tiered community project grant request for proposals (RFP), and 2). A Community Capacity Building Fund (CCBF) RFP to support agencies that want to provide Health-Related Social Needs services and be reimbursed through Medicaid.

The multi-tiered spring RFP supports local organizations in addressing the social determinants of health and equity in Yamhill County. The CCBF funds, up to $3.1 million, support local agencies’ ability to provide climate devices, housing support, nutrition assistance, and tailored outreach and education to YCCO members.

How to Apply:
For detailed information on eligibility criteria, application requirements, the funding application process, and view past webinars, local organizations are encouraged to visit our website at ycco.madcollective.com/community-programs/funding-opportunities/

Organizations seeking guidance in submitting their applications are encouraged to attend YCCO’s Joint Funding Webinar 2024 on Tuesday, March 26, from 9:30 a.m. to 11 a.m. The webinar will cover an overview of funding and application requirements and include a question-and-answer portion.
