Yamhill Community Care (YCCO) Expands Health-Related Social Needs (HRSN) Benefit to Address Barriers to Housing
McMinnville, OR — Yamhill Community Care (YCCO) is improving members’ overall well-being by addressing barriers to health through the Health-Related Social Needs (HRSN) Benefit services.
What are Health-Related Social Needs (HRSN)?
YCCO’s HRSN Benefit addresses barriers to health, such as housing instability, food insecurity, and lack of access to critical resources for Oregon Health Plan (OHP) members who need support to maintain their health — helping bridge the gap between healthcare and daily needs.
Housing Services
Starting now, launched on November 1, 2024, YCCO will provide housing support to eligible members at risk of losing housing. This includes assistance with rent and utilities, safety home-modifications, and more to maintain housing.
Climate Services
Climate device services will continue to ensure eligible members can stay safe in their homes during this upcoming Fall and Winter season. YCCO began offering climate-related support in March of this year. This includes providing health-related air conditioners, heaters, air filters, portable power supplies, and mini-fridges for medication.
Who Is Eligible?
Members of the OHP with an eligible medical condition who have had one or more of the following experiences:
- Adults and youth recently incarcerated within the past 12 months.
- Adults and youth discharged from eligible behavioral health and substance use disorder facilities, or inpatient psychiatric units within the past 12 months.
- Individuals currently or previously involved in the Oregon child welfare system.
- Individuals transitioning from Medicaid-only to dual eligibility (Medicaid and Medicare) within the past nine months or the upcoming three months.
- Individuals who are at risk of becoming unhoused.
Each benefit has different requirements, and may factor income, age, and health conditions.
