Yamhill Community Care (YCCO) Opens Applications for Health-Related Social Needs (HRSN) Community Capacity Building Funds (CCBF)

McMinnville, Ore. – Yamhill Community Care (YCCO) is pleased to announce its HRSN CCBF 2025 is now open, offering more than $1.8 million in funding available to support local nonprofits and organizations interested in enrolling as Medicaid providers to deliver services addressing social needs in climate, housing, nutrition, and outreach and engagement.

YCCO has identified priority areas for funding:

  • Outreach and engagement providers
  • Nutrition education providers
  • Housing providers

“These funds represent a significant investment in our community’s ability to address social determinants of health,” said Seamus McCarthy, President & CEO. “We are committed to supporting providers who are dedicated to making a lasting impact on the health and well-being of our members.”

Eligible nonprofits, government agencies, Tribes, and publicly funded organizations are encouraged to apply for funding to support initiatives that address health and social needs in Yamhill County and surrounding areas. The deadline to apply is May 30, 2025.
