Local Integration Family Team (LIFT)

Where needs meet solutions

Yamhill County community organizations work together to meet the immediate needs of local families who have children under 18 or are going through family reunification. The main goal is to promote sustainable health and overall well-being across the entire region.

Community members understand local needs best

The Local Integration Family Team (LIFT) meets once a month to identify barriers by working with local partners and creating solutions that help remove obstacles for the families we serve. The LIFT team includes people from schools, partner organizations, social service agencies, local government, faith groups and community leaders.

LIFT helps when no other option exists

Sometimes families need extra help with big costs like rent or heating bills during the winter. Unexpected expenses, like a medical bill or an urgent prescription, can quickly use up a family’s budget. LIFT is there to help cover these kinds of urgent costs so families can stay on track.

Needs are varied and unique for each family

Since every family’s situation is unique, LIFT provides financial support to families when no other options are available. These funds have a maximum benefit of $300, and are meant to help with a one-time crisis in one of the following areas:

  • Housing: Rental assistance, security deposit, evictions, utility assistance 
  • Food Assistance: grocery card
  • Protection from climate events: Devices that assist with maintaining healthy temperatures and clean air that are not covered under Medicaid or HRSN program
  • Therapeutic Tools: Sensory items, weighted blankets, white noise machine, noise-canceling headphones, sand tables
  • Non-Medicaid Covered Services: Prescriptions, co-pays, rides to medical and dental appointments, etc.
  • Transportation Assistance: Gas cards, bus passes, tires, registration fees
  • Items that both relieve family stress and give opportunity for positive skills practice: Zoo or museum passes, aquarium or pool passes, membership to gym or recreation center
  • Items to support gender identity and sexual orientation: Gender appropriate clothes shopping, haircuts and/or hair removal.

Service Integration Teams are made possible through a partnership with Yamhill County Health and Human Services and the support from additional funders.

Funding for LIFT

LIFT services are supported by two primary funding sources, each committed to a shared mission of promoting equitable, culturally responsive, and family-centered engagement:

  • Systems of Care (SOC): Allocates grant funds specifically for youth and family engagement initiatives, with a strong focus on inclusion.
  • Department of Early Learning and Care (DELC): Contributes funding with a mission to foster coordinated, culturally appropriate, and family-centered services, ensuring alignment across all programs.

This collaboration allows LIFT to provide comprehensive support while prioritizing inclusive and accessible resources for all families served.

How to Request Assistance from LIFT

For individual or families in need of assistance from LIFT, please contact a local community organization such as:

  • Medical provider
  • School staff
  • Government agencies
  • Law enforcement
  • Non-profits
  • Local Church
  • Tribal members

LIFT partners will work closely with you to understand your needs, collect the necessary documents and apply on your benefit.

For community partners requesting assistance on behalf of their community members. Please begin your request by completing the 2026 LIFT Application Request Form

LIFT meetings usually take place once a month to review requests, though time sensitive cases may be handled sooner. To speed up the process, please ensure all required documents are included with your application.

If your request is approved, payment typically takes about two weeks. We prefer paying via credit card; if your vendor does not accept credit card payments, you must obtain a completed W-9 from the vendor and upload it to the LIFT portal when you apply. Once the accounting department assigns a tracking number, the LIFT Specialist will email community partner for their record.

For requests to be considered time sensitive and reviewed outside of the monthly LIFT meeting, it must meet one of the following criteria below:

  • An eviction notice has been served due to unpaid rent, deposit, or mortgage payment.
  • A time sensitive item or purchase is needed that would help secure employment or housing/shelter.
  • A medical condition exists that requires immediate resolution of a problem.
  • A utility shut-off notice has been served for unpaid bills.
  • Life sustaining prescriptions or lice treatment/kits for a lice outbreak are needed.
  • Any other time-sensitive issue that puts the client/family in imminent danger and/or puts their health at risk.

If a request meets the time sensitive guideline, it will be reviewed during the hours of 8 a.m. to 5 p.m. Monday through Friday. Final approval for time sensitive requests will be completed by YCCO Early Learning Director.
