Community Information Exchange (CIE)

We are excited to share that in partnership with Unite Us, YCCO participates in the Connect Oregon collaborative providing a common infrastructure connecting health, human, and social service organizations.

Along with our many community partners, including you, we will continue to expand the coordinated care network in Yamhill, Marion, Polk, and Washington Counties that connects community-based organizations, healthcare providers, and public agencies through Unite Us’ shared technology platform.  

Connect Oregon allows us to enhance our collective efforts to address social needs that have an impact on individuals’ health and well-being across our counties.
