2016 Alternative Payment Model Applications

Yamhill CCO is committed to primary care renewal in order to support achieving the goals of Oregon’s health care transformation and the Triple Aim (improving the patient experience of care; improving the health of populations; and reducing the per capita cost of health care). This application describes the five available funding strategies and questions for each funding strategy area.

Yamhill CCO’s five APM strategies are:

  1. Baseline Funding Support: Increased fee-for-service rate, available to all primary care providers
  2. Primary Care Innovation Seed Fund: one-time grants for to build Triple Aim infrastructure
  3. Incentive/Shared Savings Program: to pass on incentive payments and shared savings to providers
  4. Advanced Primary Care Support: for care innovation
  5. Integrated Primary Care Global Budget: increase flexibility by combining strategies 1 and 4
